Fortis Health is seeking a skilled Home Health LPN to provide high-quality nursing care to patients in their homes. As a Home Health LPN, you will work under the supervision of a Registered Nurse to deliver patient-centered care and support the overall well-being of our patients.
Key Responsibilities- Provide skilled nursing care to patients in their homes, as ordered on the plan of treatment and nursing care plan.
- Evaluate the significance of assessment findings and communicate pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion.
- Assess and reassess pain, utilizing appropriate pain management techniques, and educate patients and families regarding pain management.
- Demonstrate knowledge of medications and their correct administration based on the patient's age and clinical position.
- Follow the seven medication rights and reduce the potential for medication errors.
- Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Function as a team member, demonstrating collaboration with, and responsibility to, patients, physicians, and the interdisciplinary team.
- Adhere to all HHA, payroll, billing, and documentation policies and procedures.
- Participate in HHA's other mandated activities, as requested by the Clinical Supervisor or Patient Care Services Director, including in-services and Upper Respiratory (UR)/Perfusion Index (PI) committee.
- Perform patient care responsibilities considering the needs of patients to the standard of care for patients' age.
- Articulate HHA grievance/concerns procedure.
- Articulate HHA adverse event/incident reporting procedure.
- Articulate organization's emergency management plan.
- Assist the physician and registered nurse to perform specialized procedures.
- Work as part of the interdisciplinary team.
- Complete patient documentation on time.
- Attend in-services and staff meetings as needed.
- Complete assigned training on time.
- Complete annual education requirements.
- Maintain patient confidentiality at all times.
- Perform other duties as assigned.
- Graduate of an accredited nursing program.
- Current state licensure as a LPN.
- Experience as an LPN commensurate with one of the following:
- One (1) year experience in an acute care setting within the last 24 months;
- Six (6) months home care experience within the last 12 months.
- Prior home healthcare experience is preferred.
- Valid state driver's license and reliable automobile, current automobile insurance, and being willing to operate personal car(s) necessitated by the nature of the job.
- Current TB testing.
- Current CPR card.
- No findings relating to client abuse, neglect, or misappropriation of client property.
