- Supervise and mentor LPNs and Home Health Aides to ensure seamless care coordination and patient outcomes.
- Document patient progress and outcomes using our electronic health record system, ensuring accurate and timely reporting.
- Collaborate with our team to achieve patient-centered outcomes and goals, promoting a culture of excellence and compassion.
