Responsibilities
  • Perform comprehensive patient assessments and develop individualized care plans in collaboration with our multidisciplinary team of healthcare professionals.
  • Supervise and mentor LPNs and Home Health Aides to ensure seamless care coordination and patient outcomes.
  • Document patient progress and outcomes using our electronic health record system, ensuring accurate and timely reporting.
  • Collaborate with our team to achieve patient-centered outcomes and goals, promoting a culture of excellence and compassion.
