Position Title: Case Manager at Community Partnership for Families of San Joaquin
This role is essential in directing and coordinating a variety of services offered by a community-focused family resource center. The Case Manager is tasked with ensuring the seamless integration of services from initial referral through to aftercare and case closure.
Key Responsibilities:
- Execute case management and administrative duties as part of a collaborative service team
- Ensure the maintenance of precise case records and engage in data collection efforts
- Work in partnership with informal service providers and promote community-based initiatives
- Support the overall operations of the family resource center
- Engage in service integration training and cross-training opportunities
- Deliver presentations and conduct outreach initiatives
Qualifications:
- Authorized to work in the United States
- Possess a valid driver's license and have access to transportation
- Successfully pass a background check
Education and Experience:
A college degree or equivalent experience in social work or a related discipline
Preferred Skills:
Bilingual proficiency in Spanish or other specified languages
Compensation Details:
Starting salary range from $17.00 to $19.00 per hour, accompanied by full-time benefits
Work schedule: Monday to Friday, 8:00 a.m. to 5:00 p.m.
