The Salvation Army is seeking a skilled Case Manager to join our team in providing essential support services to shelter residents. As a Case Manager, you will be responsible for coordinating the intake and interviews of shelter guests, providing resources and referrals to appropriate services, and assisting clients in finding needed services to break out of homelessness.

Key Responsibilities:
  • Coordinate the intake and interviews of shelter guests
  • Provide resources and referrals to appropriate services, including housing, employment, mental and physical health services, and more
  • Assist clients in finding needed services to break out of homelessness
  • Keep accurate records and input data into the Well Sky System
  • Ensure that all Pathway of Hope training is assigned and tracked correctly in the system
  • Provide local transportation as needed
Requirements:
  • AA or BA Degree: two to four years related experience and/or training; or equivalent combination of education and experience
  • Must be familiar with the social service aspects of The Salvation Army
  • Must have reliable transportation
  • Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
  • Must be 21 years or older
