Transform Lives as a Case Manager III

Telecare Corporation seeks a dedicated Case Manager III to provide primary support and case management services to individuals with mental illness and co-occurring disorders. As a key member of our team, you will assist clients in living independently in their chosen community.

This role is distinguished by additional education, years of experience, and the ability to proficiently perform job functions and meet position standards.

Key Responsibilities:

  • Under direct supervision, serve as a primary support and case manager for individuals with mental illness and co-occurring disorders.
  • Assist clients in achieving their goals and living independently in their chosen community.
  • Develop and implement individualized treatment plans in collaboration with clients, family members, and other healthcare professionals.
  • Provide crisis intervention and support services as needed.
  • Collaborate with community resources and service providers to ensure seamless care coordination.

Requirements:

  • Bachelor's degree in Social Services or Human Services and one year of experience or High School Graduation or G.E.D. equivalent and four years of direct services in Mental Health or community services.
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups.
