Job Summary

We are seeking a skilled and compassionate RN Home Health and Hospice Care Specialist to join our team at SSM Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in their homes and community-based settings.

Responsibilities
  • Assess and coordinate patient care needs with the healthcare team, ensuring timely and effective delivery of services.
  • Perform point-of-care testing and provide skilled nursing care within established plans of care, including vital sign monitoring, wound care, IV management, and physical assessments.
  • Collaborate with patients, families, and healthcare teams to develop comprehensive plans of care, addressing unique patient needs and goals.
  • Document visit notes, treatments, and care, ensuring accurate and timely completion of required documentation.
  • Respond to patient and family inquiries, addressing needs, issues, or concerns in a timely and compassionate manner.
Requirements
  • Graduate of an accredited school of nursing or equivalent education for licensure.
  • Current RN licensure in the state of practice (Illinois, Missouri, Oklahoma, or Wisconsin).
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Valid driver's license and reliable transportation.
