Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health and Hospice team as a Clinical Coordinator. In this role, you will be responsible for coordinating and facilitating the interdisciplinary plan of care for patients in their homes, community-based residential facilities, assisted living locations, hospitals, or other settings.

Key Responsibilities
  • Assess and coordinate patient discharge planning needs with the healthcare team.
  • Perform point-of-care testing according to policies and procedures.
  • Collaborate with physicians to develop and implement patient care plans.
  • Provide skilled nursing care within the established plan of care, including vital sign monitoring, wound care, IV management, and physical assessments.
  • Document visit notes, treatments, and care in a timely and accurate manner.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • Graduate of an accredited school of nursing.
  • Current RN licensure in the state of practice.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and reliable transportation.
