Job Title: Habilitation Specialist

Constructive Partnerships Unlimited (CP Unlimited) is seeking a skilled Habilitation Specialist to join our team at our Queens Long Island City Day Program. As a Habilitation Specialist, you will play a vital role in implementing and monitoring program plans for individuals with intellectual and developmental disabilities.

Key Responsibilities:

  • Implement and monitor program plans for individuals with intellectual and developmental disabilities.
  • Ensure daily documentation is maintained and uses various software for data collection.
  • Provide direction to Habilitation Assistants and ensure the health, safety, and well-being of all individuals attending the program.
  • Notify management and/or nurse of any medical concerns and accompany individuals to the hospital and/or healthcare provider as needed.

Requirements:

  • Associates Degree/60 credits in a Health Care or Educational field.
  • 1 year experience working with individuals with intellectual and/or developmental disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to perform data entry on a personal computer (PC). MS Word a plus.
  • Criminal background check.
  • Valid NYS Driver's License preferred.
