Job Summary

Front Porch is seeking a highly skilled and dedicated MDS Coordinator to join our team. As an MDS Coordinator, you will play a critical role in ensuring the delivery of high-quality care to our residents. Your primary responsibility will be to conduct and coordinate the completion of the resident comprehensive assessment (RCA), which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet.

Key Responsibilities
  • Conduct and coordinate the completion of the resident comprehensive assessment (RCA)
  • Ensure interdisciplinary, comprehensive assessments and care plans are completed within established timeframes
  • Schedule and facilitate interdisciplinary conferences for review and updating of resident assessments and care plans
  • Monitor plans of care to ensure resident assessments are utilized in the development of an interdisciplinary plan of care reflective of the resident's current physical, mental, and psychosocial needs
  • Provide educational training and clinical supervision in resident assessment for appropriate healthcare workers
  • Assist Resident Assessment Coordinators (MDS) with computerized medical data and transmission
  • Ensure documentation relative to resident assessment process (RAI) is maintained according to State and Federal regulations and company policy and procedure
  • Communicate problem identification with resident assessment process promptly to the DON and Executive Director
  • Conduct and participate in PDPM meetings
  • Report data collection, trends, QSAP reports, and attend QA meetings
Requirements
  • Registered Nurse - California
  • Graduate from accredited school of nursing
  • 0 Year Experience/training as required by state regulations
