Job Summary

We are seeking an experienced Oncology Nurse Navigator to join our team at Stanford Health Care. As a key member of our cancer care team, you will play a critical role in ensuring that patients receive the highest quality care and support throughout their cancer journey.

Key Responsibilities
  • Provide clinical care coordination and navigation to patients with cancer, including assessment of their needs and development of individualized care plans.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordination of services.
  • Provide education and support to patients and their families, addressing their physical, emotional, and spiritual needs.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Minimum of 5 years of nursing experience, with at least 2-3 years in oncology.
  • Experience with nurse navigation and/or ambulatory oncology case management is preferred.
  • Strong clinical knowledge of cancer disease processes and treatment options.
  • Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
