About the Role

We are seeking a compassionate and skilled Hospice Care Coordinator to join our team at Enhabit Home Health & Hospice. As a key member of our interdisciplinary care team, you will play a vital role in providing high-quality patient care and support to patients and their families.

The successful candidate will have experience working in hospice or home health care, with excellent communication and interpersonal skills. They will be able to assess patients' needs, develop individualized care plans, and work collaboratively with healthcare teams to ensure seamless care transitions.

Key Responsibilities
  • Coordinate patient care services, ensuring timely delivery of quality care
  • Assess patients' and family members' needs, identifying areas for improvement
  • Develop, implement, and update individualized care plans, reflecting patients' preferences and goals
  • Collaborate with healthcare teams to facilitate effective communication and coordination of care
Requirements
  • Bachelor's degree in Nursing or related field
  • Licensed as a Registered Nurse (RN) in the state of employment
  • At least one year of experience in hospice or home health care, preferably in a similar role
