Job Summary and Qualifications:

The Clinical Nurse Coordinator (CNC) plays a vital role in ensuring the delivery of high-quality, patient-centered care in our rehabilitation department.

This position involves coordinating all aspects of patient care, including admission and discharge processes, participating in quality assessments, collaborating with subject matter experts, and supporting a patient-first philosophy.

Responsibilities:

  • Assist with efficient throughput and high-quality patient care during admission and discharge processes
  • Participate in ongoing assessment of patient care services and provide recommendations for improvement
  • Collaborate with subject matter experts to create an environment of teamwork that supports improved outcomes and service
  • Support a patient-first philosophy and engage in service recovery when necessary
  • Operationalize current nursing strategies, including employee rounding, hourly rounds, and other initiatives to improve engagement

Requirements:

  • Bachelor's degree in Nursing or Associate's degree in Nursing with relevant experience
  • Basic Cardiac Life Support certification within 30 days of employment start date
  • Registered Nurse licensure in the state of Texas
