The Clinical Nurse Care Coordinator (CNCC) at PACE Southeast Michigan (PACE SEMI) plays a pivotal role in delivering holistic nursing services to elderly individuals with intricate health requirements. This position encompasses the evaluation, strategizing, execution, and assessment of participant results, alongside effective education on health maintenance and intervention.

KEY RESPONSIBILITIES:

  • Evaluate the needs of participants and formulate suitable care strategies.
  • Collaborate with participants, their families, and a diverse team of healthcare professionals.
  • Enhance participants' functional abilities and promote independence.
  • Provide oversight, guidance, and education regarding nursing care necessities.
  • Administer medications and treatments, observe reactions, and alert medical staff of any changes.
  • Initiate preventive and rehabilitative measures as required.
  • Assess participant outcomes and communicate with the team for modifications to care strategies.
  • Maintain precise records and documentation in accordance with established policies and procedures.
  • Advocate for participants and ensure high satisfaction levels.

QUALIFICATIONS:

  • Registered Nurse with active Michigan licensure (BSN preferred).
  • Prior experience working with elderly or frail populations.
  • Ability to foster interpersonal relationships and lead nursing personnel.
  • Commitment to ongoing professional development through education and training.
  • Valid Michigan driver's license with a satisfactory driving record.

WORK ENVIRONMENT:

  • Operates in participants' residences in varied settings.
  • Potential exposure to infectious materials and hazardous substances.
  • Requires travel within the designated area with exposure to varying weather conditions.
  • Involves frequent physical activities such as walking, bending, and lifting.
