Job Summary:

We are seeking a dynamic Behavioral Health Specialist to join our team at Primary Health Solutions. The ideal candidate will have a master's degree in social work or Clinical Mental Health Counseling and be licensed in the state of Ohio.

Key Responsibilities:

Provides clinical services for referred patients in school and/or clinic settings.
Offers consultation to area schools and agencies.
Provides treatment and/or care coordination of mental health issues.
Conduct clinical assessment.
Develop and implement integrated wellness plan consistent with principles of client self-management.
Facilitate behavioral interventions.
Crisis interventions.
Collaborate with agency staff and community service providers to coordinate referrals and delivery of services, and complete discharge plans.
Provides training skills and patient education strategies and develops specific behavioral change plans for patients and behavioral health protocols.

Requirements:

Active master's degree in social work or Clinical Mental Health Counseling required.
Active Licensure in the state of Ohio required.
Must have graduated from an approved social work or counseling program.
Two years' experience (preferred) working with indigent populations and community-based programs.
Experience as a Behaviorist or in a similar role
Working knowledge of NextGen or other EHR/EMR systems
Ability to handle confidential information
Problem solver that can adapt to changes
Attention to detail
