Key Responsibilities
  • Provide crisis intervention, service linkages, and discharge planning to individuals with mental illness
  • Develop treatment plans with individuals and treatment teams, monitoring and evaluating progress, and modifying treatment plans as needed
  • Determine discharge needs and appropriate community resources
  • Establish and maintain relationships with community providers to ensure continuity of care
Requirements
  • Must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW)
  • Must be able to work collaboratively as part of a multidisciplinary team
  • Must have excellent communication and interpersonal skills
