Social Worker

We are seeking a skilled Social Worker to join our Home Care team at Northwell Health. As a Social Worker, you will provide individual and group work services to patients and their families in various healthcare settings.
Key Responsibilities- Deliver culturally competent direct services to patients and their families using various social work modalities.
- Identify patients in need of or who could benefit from social work assessment and intervention using high-risk criteria and/or consultation referrals.
- Advocate on behalf of patients to ensure maximum access to healthcare, assess barriers, and offer solutions.
- Provide interdisciplinary teams with assessment findings and suggest potential impact on overall patient care.
- Develop and implement treatment and discharge plans that are safe and timely, promoting patient choice.
- Initiate communication with post-acute providers and payers as needed.
- Incorporate knowledge of community resources and entitlement programs in all planning.
- Maintain appropriate professional patient care documentation as defined by site policies and procedures.
- Represent the department and profession in various hospital/site-based committees and work groups.
- May assign and monitor undergraduate and graduate-level Social Work students, providing supervision and evaluation.
- Master of Social Work degree required.
- Current License to practice as a Master Social Worker or Clinical Social Worker in New York State required, plus specialized certifications as needed.
- 1-3 years of relevant experience required.
