Position Overview

The Family Support Advocate for Infant Care plays a crucial role in connecting families with essential resources and support systems. This position involves collaborating with various community partners to enhance maternal and infant health outcomes.

Required Qualifications

  • Education: High school diploma or GED.
  • Work Experience: Proficiency in using databases, email, internet applications, and virtual communication platforms such as Zoom and Teams. Bilingual proficiency in Spanish is required.

Preferred Qualifications

  • Education: Bachelor's Degree in health, social work, psychology, or a related discipline.
  • Experience: A minimum of 2 years of experience in community-based programs, healthcare systems, health departments, or clinics. Familiarity with maternal and infant health resources is advantageous. Experience with data collection and management, as well as knowledge of the KanCare system and the uninsured population, is preferred.
  • Skills: Strong interpersonal skills with the ability to engage effectively with diverse individuals, both in-person and virtually. Capability to build long-lasting relationships with key stakeholders in partner organizations. Must maintain confidentiality and demonstrate excellent organizational skills. Critical thinking abilities are essential for problem-solving.

Key Responsibilities

  • Act as the primary liaison among partnering organizations, volunteers, clients, and the program team.
  • Promote the program actively within community outreach initiatives.
  • Manage referral processes, including tracking and follow-up.
  • Coordinate logistics for both in-person and virtual classes, ensuring all necessary materials are prepared.
  • Maintain client profiles and oversee attendance to ensure program completion.
  • Utilize standardized communication methods to remind participants of class schedules.
  • Provide regular updates to instructors and administrative staff regarding program developments.
  • Stay informed about community resources to assist clients effectively.
  • Maintain a consistent presence at clinical locations to support program initiatives.
  • Assist with statewide program classes and participant needs as required.
  • Identify and address barriers to client participation in the program.
  • Facilitate connections to additional community resources as needed.

  • Secure necessary authorizations and consent from clients.
  • Collect and enter program-related data accurately.
  • Participate in regular team meetings and training sessions.
  • Assist in preparing reports and grant applications as needed.
