Candidate Profile

As a Registered Nurse Clinical Advocate, you will:

Report directly to the AVP of Business Development and be part of a dedicated team responsible for:

  • Ensuring efficient and timely intake and referral processes to meet the needs of patients and families.
  • Completing admissions for referred patients in urgent situations, as staffing allows.
  • Providing accurate information about hospice services to inquiries.
  • Responding promptly to calls from referral sources and initiating the intake process.
  • Acting as a patient advocate in developing care plans and obtaining informed consent for hospice services.
  • Engaging with various account types, including Skilled Nursing Facilities, Assisted Living Facilities, Hospitals, and Physicians.
  • Conducting educational sessions on the advantages of hospice care.

Qualifications for the Role:

  • A compassionate and empathetic approach to patient care.
  • Active RN license in the relevant state(s).
  • A minimum of 2 years of experience in healthcare, preferably in hospice, home health, clinical liaison, or related fields.
  • A strong reputation within the community.
