About the Role

VNS Health is seeking a skilled Registered Nurse (RN) to lead our Hospice Team in Brooklyn. As a Hospice Care Manager, you will be responsible for overseeing the delivery of high-quality end-of-life care to patients and their families in the comfort of their own homes.

Key Responsibilities
  • Lead a dedicated interdisciplinary team in providing compassionate and patient-centered care
  • Ensure regulatory compliance and quality improvement initiatives are implemented
  • Collaborate with Quality Improvement and Education to ensure professional practice standards are met
  • Manage day-to-day operations, including caseload and visit management, productivity, and revenue/expenditure targets
Requirements
  • License and current registration to practice as a Registered Professional Nurse in New York State
  • Bachelor's degree in nursing from an approved program
  • Minimum three years of clinical nursing experience, preferably in a home care or hospice setting
  • Clinical management experience preferred
