Job Title: Home Health Speech Language Pathologist

VNA Health Group is seeking a skilled Speech Language Pathologist to join our team in Ridgefield, New Jersey.

Job Summary:

We are looking for a dedicated and compassionate Speech Language Pathologist to provide high-quality speech and language therapy services to patients in their homes or alternate community settings.

Responsibilities:
  • Evaluate and assess clients for speech and language therapy needs, developing and implementing individualized plans of action in collaboration with physicians.
  • Provide speech and language therapy services to clients, monitoring progress and making adjustments to treatment plans as needed.
  • Document all changes in client condition and update care plans and outcomes accordingly.
  • Identify counseling needs and plan appropriate teaching strategies to support clients and their families.
  • Monitor quality and appropriateness of services provided, adhering to agency infection control and safety policies.
Requirements:
  • Licensed Speech-Language Pathologist with CCC-SLP certification and Master's degree in Speech-Language Pathology.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and effective communication and interpersonal skills.
  • Valid NJ driver's license.
