Job Description

The Primary Care Physician will deliver high-quality primary health care and patient care management in a residential setting. Responsibilities include delivering primary care to high-acuity patients in their homes, Monday through Friday.

Qualifications
  • Geriatrics or complex patient experience preferred.
  • Active/unrestricted medical license and DEA required.
  • Board Certified or Board Eligible.
  • EMR experience necessary.
