Job Overview

HousingPlus, Inc. seeks a highly skilled and compassionate Social Worker to join our team as a Permanent Housing Case Manager. In this role, you will play a critical part in supporting individuals who reside in permanent supportive housing for women and women with children who have experienced chronic homelessness, mental illness, or substance use disorder.

The ideal candidate will possess a Master's Degree in Social Work or a related field and at least two years of experience working with vulnerable populations. Excellent communication and interpersonal skills are essential, along with the ability to work independently and as part of a team. Key Responsibilities
  • Needs Assessment and Service Planning: Conduct initial and ongoing needs assessments, develop individualized service plans, and review program information with new tenants.
  • Casework and Case Management: Meet regularly with tenants to discuss goals, obtain required signatures on service plans, and maintain accurate documentation of services and progress.
  • Community Referrals and Partnerships: Provide referrals to community-based agencies and resources, escort tenants to appointments as needed, and facilitate group sessions and special support projects.
  • Supervision and Training: Participate in individual supervisory meetings, case reviews, and staff training as required.

We offer a competitive salary range of $55,000 - $57,000, a 35-hour workweek, and an attractive benefits package including medical, dental, vision, commuter benefits, and supplemental insurance options.
