Key Responsibilities
  • Coordinate with Intake Specialists to complete PRAPARE assessments for new and annual patient visits
  • Conduct PRAPARE assessments for all clients enrolled in primary care and behavioral health, facilitating overall coordination of care and case management referrals
  • Collaborate with other care management teams internally and externally to help patients meet their care plan goals
  • Facilitate enrollment of patients in the Undetectables program and ensure patients remain in good standing

The PCC will also participate in interdisciplinary meetings, transcribe meeting minutes, and oversee documentation of hospitalizations and discharge follow-up appointments.
