Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Home Health Registered Nurse - Case Manager. As a key member of our care team, you will be responsible for managing and delivering patient care plans, collaborating with healthcare professionals, and providing exceptional patient care and support.

Key Responsibilities
  • Manage and deliver patient care plans, ensuring high-quality care and outcomes
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to coordinate patient care
  • Conduct thorough assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans, taking into account patients' goals, preferences, and values
  • Provide education and support to patients and their families, promoting self-care and independence
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Communicate effectively with patients, families, and healthcare teams, ensuring seamless care coordination
Requirements
  • Current RN license in the state of Illinois
  • One year of experience as a Registered Nurse, with a strong background in home health care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care
  • Current driver's license and reliable transportation
