About the Role

We are seeking a highly skilled and compassionate Home Health Registered Nurse to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive skilled nursing visits to patients in their homes, including admission, case management, and follow-up care.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides.
  • Monitor patient progress, report to physicians and clinical managers, and revise care plans as needed.
  • Supervise and educate home health aides and personal care workers to ensure high-quality care delivery.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
