As a Clinical Assessment Advisor, you'll play a vital role in our team, working closely with patients, physicians, and other healthcare professionals to ensure seamless care transitions and optimal clinical outcomes.

Key Responsibilities:

  • Conduct thorough patient assessments to identify needs and develop personalized care plans
  • Collaborate with interdisciplinary teams to coordinate care and ensure timely interventions
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
  • Monitor patient progress, making adjustments to care plans as needed to achieve desired outcomes
  • Participate in quality improvement initiatives to enhance patient care and satisfaction

Requirements:

  • Bachelor of Science degree in Nursing (BSN) or equivalent experience
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role
  • Current CPR certification and ability to maintain licensure
  • Excellent communication, interpersonal, and problem-solving skills
  • Knowledge of state and federal home health regulations and compliance standards
  • Ability to work in a fast-paced environment and prioritize multiple tasks
