We are seeking a highly skilled Clinical Liaison to join our team at PruittHealth. As a key member of our organization, you will play a vital role in facilitating referrals and admissions to our home health and hospice agencies.
Key Responsibilities:- Assess patients to ensure they are suitable for admission to our home health or hospice agency
- Provide clinical expertise to our agencies by evaluating their ability to meet patient needs
- Develop and sustain business partnerships with hospitals and community services to drive referrals
- Stay up-to-date with regulatory agencies, reimbursement systems, and levels of care
- Communicate effectively with patients, families, and healthcare professionals
- Take responsibility for a regional territory of hospitals and community services to facilitate patient placements
- Provide excellent customer service to referral sources and network providers
- Represent PruittHealth in community efforts and education seminars
- 2 years of college or business school, or equivalent experience and training
- 2 years of experience in home health and/or hospice, or equivalent combination of experience and training
- RN or LPN license, with a Bachelor's Degree preferred
