About the Role

VNA Health Group is seeking a skilled Physical Therapist to join our team in Independence, Ohio. As a Physical Therapist, you will play a vital role in providing home care therapy to individuals, helping them restore function, improve mobility, and relieve pain.

Responsibilities
  • Conduct initial assessments to determine patient treatment needs and develop personalized plans of care.
  • Provide direct physical therapy services, including performance of physical therapy procedures, instruction of patients and caregivers, and recommendations for equipment or adaptation of home environments.
  • Re-evaluate and revise plans of care as needed with physician approval.
  • Facilitate total care services to patients by making appropriate referrals to other disciplines and obtaining physician orders or consulting with case managers.
  • Submit all documents in a timely manner and obtain physician approval to modify current plans of care or treatment programs.
Qualifications
  • Bachelor's Degree in Physical Therapy preferred.
  • One year of experience in a supervised healthcare setting.
  • Ability to work effectively with diverse cultural, social, and ethnic backgrounds and collaborate in a complex institutional structure.
  • Strong planning and organizational skills, with the ability to work well under pressure and maintain attention to detail.
  • Ability to work independently with minimal supervision and maintain confidentiality.
  • Licensed Physical Therapist by the State of Ohio, CPR certification, and a valid Driver's License.
