Qualifications:

  • Formal education as a Registered Nurse or equivalent experience (RN, LVN, LPN)
  • Exceptional verbal and written communication skills in English
  • Comprehensive knowledge of disease processes related to vital sign monitoring, including hypertension, heart failure, chronic kidney disease, asthma/COPD, and obesity
  • Strong time management and prioritization abilities
  • Proficient assessment and critical thinking skills
  • Clinical experience in inpatient environments is preferred
  • Experience in adult care or pediatrics is advantageous

Key Responsibilities:

  • Conduct remote physiologic monitoring treatment management services, requiring interactive communication with patients and caregivers
  • Perform verbal assessments and provide health education to patients
  • Manage online guidelines to address customer inquiries
  • Coordinate patient care and collaborate with healthcare team members through electronic medical records
  • Monitor vital signs and assess whether they fall within acceptable ranges
  • Evaluate patient vitals and prioritize care based on individual needs
  • Maintain patient confidentiality and uphold patient rights in a digital environment
  • Continuously monitor and assess patient progress, responding to alerts through remote monitoring tools
  • Handle patient consultation calls and inquiries

Expectations:

  • Be available, accessible, and accountable during scheduled work hours
  • Adhere to established support processes and procedures
  • Comply with organizational security and compliance standards, including training and performance management
  • Possess an active Compact License
  • Hold a valid US Nursing License
