Key Responsibilities
  • Evaluate and Assess: Conduct thorough assessments of patients' nutritional status to determine appropriate nutrition interventions and recommendations.
  • Develop and Implement: Create individualized Medical Nutrition Therapy (MNT) plans for patients based on lab results and individualized goals.
  • Provide Education and Counseling: Select and develop nutrition education and counseling programs tailored to patients' needs, including nutrient requirements, knowledge, and abilities.
  • Monitor and Evaluate: Continuously monitor patients' nutritional status, laboratory values, and response to nutrition-related medications and therapy to determine the effectiveness of interventions.
  • Collaborate and Communicate: Effectively communicate with healthcare teams, patients, and caregivers to ensure seamless care coordination and patient outcomes.
Requirements
  • Education and Certification: Bachelor's degree in Nutrition, Dietetics, or a related field, and registration as a Registered Dietitian.
  • Experience and Skills: Minimum 1 year of clinical experience, with intermediate computer skills and proficiency in MS Office and DaVita-specific applications.
  • Travel Requirements: Travel up to 30% as needed to meet patient needs, attend meetings, and provide cross-coverage support.
