Job Overview

Your Health Organization is seeking a skilled Transitional Care Professional to join our team. In this critical role, you will play a vital part in ensuring a seamless transition for patients from hospitals, rehabilitation centers, and skilled nursing facilities to their homes.

Key Responsibilities
  • Visit patients in assigned facilities to establish trust and rapport, providing guidance and feedback to assist them in making informed decisions about their health.
  • Coordinate a smooth transition for patients returning home by arranging necessary services, such as home health, outpatient rehab, and follow-up care with our providers and support staff.
  • Schedule and coordinate essential visits, including those with our primary care provider, specialists, and other healthcare professionals involved in the patient's care.
  • Ensure effective communication between healthcare professionals and facilitate patient and family adherence to the plan of care.
  • Provide exceptional customer service, interacting with patients via phone, email, patient portal, and other means to gather necessary information.
Requirements
  • Certified Medical Assistant (CMA) certification, with a licensed nurse preferred.
  • A minimum of three years of clinical experience, with demonstrated ability to supervise and direct professional and administrative personnel.
  • Strong written and verbal skills, with ability to read and communicate effectively.
  • Basic computer knowledge and ability to manage multiple responsibilities, with sound judgment.
  • Ability to work flexible hours, travel between offices, and possess a valid driver's license with an insured vehicle.
Work Environment

The work environment for this role may involve exposure to housekeeping cleaning agents and chemicals, humidity, hot equipment, and/or noise. Additionally, you may be exposed to infections, communicable diseases, odors, bloodborne pathogens, excreta, and hazardous materials.

You will be expected to comply with all federal, state, and local laws and regulations, as well as our organization's policies and procedures. This includes knowledge of and compliance with the Patient's Bill of Rights, Medicare guidelines, and our Code of Conduct/ Ethics.

Physical Demands

The physical demands of this role include the ability to lift, reach, bend, push, pull, use repetitive hand motions, walk, and carry simultaneously. You must also be able to withstand reaching, stooping, bending, kneeling, and crouching, as well as walking and standing for periods of time, and lifting up to 50 pounds.
