Job Summary:The Community Liaison serves as an outreach worker to identify hard to reach, vulnerable and underserved populations. He/she will develop and implement community outreach activities to ensure that individuals are promptly connected to primary care and other specialty services at Gotham Health through our Federally Qualified Health Centers (FQHCs) and Community Health Centers. The Community Liaison will work collaboratively with community-based organizations, educational partners, government agencies as well as internal stakeholders across Gotham Health and facilities across our health system (H+H). The Community Liaison will be responsible, in part, for patient growth and referral generation through strategic partnership engagement, increasing new business across Gotham Health.
Key Responsibilities:- Assists with establishing and maintaining positive relationships with community partners, faith-based organizations, government agencies, advocacy groups that target vulnerable populations, and performs other activities to enhance Gotham Health's presence in all NYC communities;
- Conducts in-service presentations and performs other activities in an effort to maintain and establish positive relationships with internal and external stakeholders;
- Attends meetings with local community boards across the five boroughs and represents Gotham Health at various community events to promote, educate, and build awareness or programs and services;
- Cultivates community partnerships through routine meeting attendance, health education workshops, virtual and in-person activities to educate partners on our value proposition;
- Provides onsite support and management for Gotham Health-sponsored events and forums;
- Conducts marketing presentations to increase effectiveness in establishing rapport, assessing individual needs and communicating our value proposition;
- Maintains working relationship with local and specialty media contacts germane to the health care industry, regional business and other key outlets;
- Track productivity metrics and report bi-weekly summary report detailing planned and executed outreach efforts;
- Assist with on-the-spot scheduling of new patients during community outreach activities;
- Maintain flexible schedule to work evenings and weekends to accommodate staffing needs; Travel within the five boroughs of New York City is required, as needed;
- Provide orientation, training and development for staff;
- Supervise community affairs staff, as needed;
- Other duties as assigned.
- Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs;
- Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
- 2-3 years of relevant experience in marketing, business development, community relations, or government affairs;
- Strong communication skills (verbal and written) and confident with public speaking;
- Demonstrate sound decision-making problem solving, good judgement, research and presentation skills;
- Excellent time management and organizational skills to meet deadlines;
- Demonstrate enthusiasm for motivating people;
- Capacity to work independently and in a collaborative environment;
- Ability to use computers and mobile devices to complete moderate to complex tasks;
- Bilingual (e.g. Spanish, Chinese, Bengali, Urdu, French, Haitian Creole).
