Job Summary

Camarena Health is seeking a highly skilled and compassionate Case Manager - Patient Outreach Specialist to join our team. As a key member of our healthcare team, you will play a vital role in ensuring patients receive the necessary clinical services to improve their health outcomes.

Key Responsibilities
  • Patient Outreach and Engagement: Develop and implement effective outreach strategies to engage patients in their care, ensuring they receive necessary services and support.
  • Health Education and Support: Provide patients with education and support to help them understand and manage their health conditions, promoting self-care and empowerment.
  • Collaboration and Communication: Work closely with healthcare providers, staff, and patients to ensure seamless communication and coordination of care.
  • Quality Improvement: Contribute to quality improvement initiatives, identifying areas for improvement and implementing strategies to enhance patient outcomes.
  • Documentation and Reporting: Accurately document patient interactions and outcomes, providing regular reports to stakeholders as needed.
Requirements
  • Education: High School Diploma or equivalent required; Medical Assistant certification or prior experience preferred.
  • Skills: Excellent communication and interpersonal skills; ability to work effectively in a fast-paced environment; proficiency in computer applications and medical terminology.
  • Experience: Previous experience in a healthcare setting, preferably as a Medical Assistant or Case Manager; familiarity with primary care outpatient clinic procedures.
Working Conditions

Must be able to move up to 20 pounds and push up to 50 pounds (on wheels); must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to patients and staff.
