Health Care Coordinator

About the Role:
The Health Navigator plays a critical role in our organization, working closely with patients, providers, and clinic staff to ensure seamless care coordination. As a Health Navigator, you will develop strong relationships with patients, assist with various administrative and logistical tasks, and help doctors focus on delivering high-quality medical care.
Your Responsibilities:
- Meet with patients during the check-in process to gather information and provide support
- Enter data into our software platform for reporting and tracking purposes
- Call patients to schedule office visits or remind them to pick up medication
- Order and schedule procedures, tests, and screenings as needed
- Identify and track progress on important gaps in patient care
- Maintain accurate records of patient interactions and communicate with providers using electronic health records
- Educate patients on their healthcare options, insurance benefits, and common medical conditions
- Help patients access community resources and coordinate follow-up care after hospital discharge
