Job Summary

We are seeking a highly skilled High Risk Case Management Social Worker to join our One Medical team. As a key member of our interdisciplinary team, you will provide comprehensive care to patients with complex medical conditions, addressing biopsychosocial barriers to their health and well-being.

Key Responsibilities
  • Conduct thorough biopsychosocial assessments to identify patient needs and develop comprehensive care plans.
  • Collaborate with patients, caregivers, and healthcare providers to overcome physical, financial, and emotional barriers to care.
  • Facilitate discussions around advanced care planning, long-term care planning, and palliative care options.
  • Link patients to community resources and services to improve health outcomes.
  • Work with our team to ensure seamless transitions of care and coordination across healthcare settings.
Requirements
  • Licensed Master of Social Work (LMSW) required with ability to achieve reciprocity to cover additional state markets within a year of employment.
  • 3+ years of experience as a Licensed Social Worker with demonstrated experience in high-risk, complex care settings, senior health, and/or case management experience.
  • Master of Social Work (MSW) required.
  • Experience with home-based care services, hospitals/SNF, and long-term care facilities preferred.
