Key Responsibilities- Conduct thorough biopsychosocial assessments to identify patient needs and develop comprehensive care plans.
- Collaborate with patients, caregivers, and healthcare providers to overcome physical, financial, and emotional barriers to care.
- Facilitate discussions around advanced care planning, long-term care planning, and palliative care options.
- Link patients to community resources and services to improve health outcomes.
- Work with our team to ensure seamless transitions of care and coordination across healthcare settings.
- Licensed Master of Social Work (LMSW) required with ability to achieve reciprocity to cover additional state markets within a year of employment.
- 3+ years of experience as a Licensed Social Worker with demonstrated experience in high-risk, complex care settings, senior health, and/or case management experience.
- Master of Social Work (MSW) required.
- Experience with home-based care services, hospitals/SNF, and long-term care facilities preferred.
