- Develop and implement individualized service plans to address patient needs and goals.
- Collaborate with medical staff and patients to coordinate care and ensure seamless transitions.
- Provide education and resources to patients and families to promote health and wellness.
- Monitor treatment compliance and make adjustments as needed.
- Participate in discharge planning to ensure a smooth transition to community-based care.
- Master's degree in social work from an accredited university or college.
- Licensed Social Worker in the state of Texas or a transferrable license from another state.
- Minimum 2 years of paid experience in case management, preferably in a primary care or family practice setting.
- Excellent written and verbal communication skills.
- Detailed-oriented and organized with strong time management skills.
- Self-motivated and able to work independently with minimal supervision.
- Strong professional ethics and boundaries.
- Ability to work effectively in a fast-paced environment with frequent interruptions.
- Bilingual (Spanish/English) a plus.
