Care Coordinator for End-of-Life Patients

We are seeking a compassionate and skilled Registered Nurse to join our team as a Care Coordinator. As a key member of our care team, you will play a vital role in ensuring that patients receive the highest quality care and support during their end-of-life journey.

Responsibilities- Evaluate patients and develop individualized care plans to meet their unique needs and goals
- Communicate effectively with patients, families, and the care team to ensure seamless coordination of care
- Provide emotional support and education to patients and their families throughout the care process
- Collaborate with the care team to ensure timely and effective implementation of care plans
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- At least 2 years of experience in a clinical care setting, preferably in hospice care
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment
- Ability to sit, stand, bend, move intermittently, and lift at least 80-100 lbs
