As a Patient Care Coordinator, you will be responsible for assisting residents with daily living activities, including bathing, dressing, and personal hygiene. You will also complete patient care records accurately and perform other duties as assigned by the charge nurse.

The ideal candidate will have a high school diploma or equivalent, be at least 18 years old, and be listed in the DFS registry as a certified nursing assistant. You should also be willing to care for geriatric patients and have a genuine concern for their welfare. We offer a competitive salary of $32,000 - $40,000 per year, depending on experience, and a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth.
