About the Role

Gentiva Hospice is seeking a compassionate and skilled Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will play a vital role in providing high-quality care to patients and their families in their homes.

Key Responsibilities
  • Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming.
  • Assist patients with ambulation and transfer to and from beds, wheelchairs, and other equipment.
  • Prepare and serve light meals and snacks, and assist with feeding patients as needed.
  • Assist with light housekeeping tasks, such as emptying trash and changing linens.
  • Report any changes in patient condition or home situation to the Registered Nurse and members of the interdisciplinary team.
Requirements
  • Certified Nursing Assistant (CNA) license.
  • At least six months of patient care experience in hospice, home health, ICU, geriatric, or oncology.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
