Hospice Care Specialist

We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at Hospice of the Piedmont. As a Certified Nursing Assistant (CNA), you will play a vital role in providing exceptional care to patients in their homes during their final stages of life.
Responsibilities:- Provide direct patient care in accordance with the care plan developed by the interdisciplinary team.
- Assist with activities of daily living (ADLs), personal hygiene, and comfort measures.
- Observe and report any changes in patient condition or behavior to the hospice nurse.
- Collaborate with the interdisciplinary team to ensure the highest quality of care.
Requirements:
- Valid certification as a Certified Nursing Assistant (CNA) in Virginia.
- Prior experience in hospice care or a similar setting is preferred but not required.
- Demonstrated knowledge of principles and practices of direct patient care.
