About the Role:As a CNA in our hospice care program, you will play a vital role in providing comfort, support, and assistance to patients and their families during their final stages of life.
Responsibilities:- Provide direct patient care in accordance with the care plan developed by the interdisciplinary team, including assistance with activities of daily living (ADLs), personal hygiene, and comfort measures.
- Observe and report any changes in patient condition or behavior to the hospice nurse.
- Assist with mobility and transfer of patients as needed.
- Document all care provided and any changes in patient condition accurately and timely.
- Provide emotional support and companionship to patients and their families.
- Collaborate with the interdisciplinary team to ensure the highest quality of care.
- Maintain a clean and safe environment for patients.
- Respect and uphold patient rights and confidentiality.
- Valid certification as a Certified Nursing Assistant (CNA) in Virginia.
- Prior experience in hospice care or a similar setting is preferred but not required.
- Demonstrated knowledge of principles and practices of direct patient care.
- Compassionate and empathetic nature with the ability to provide emotional support.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Strong organizational skills and attention to detail.
- Ability to maintain confidentiality and professionalism.
- Physical stamina to perform the duties and responsibilities of the position.
