Job Summary:
In partnership with Della Infotech, Inc., we are seeking a skilled Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) to join our dynamic team as a Clinical Care Specialist in a Sub-Acute setting.

About the Role:
This is an exciting opportunity for an experienced LVN/LPN to provide high-quality patient care and support in a fast-paced, dynamic environment. As a Clinical Care Specialist, you will be responsible for delivering exceptional patient outcomes while collaborating with multidisciplinary teams to ensure seamless care transitions.

Key Responsibilities:
• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans that cater to the unique needs of each patient
• Collaborate with healthcare professionals to coordinate patient transfers, discharges, and other clinical activities
• Participate in quality improvement initiatives to enhance patient safety and satisfaction
• Maintain accurate and comprehensive documentation of patient records

Requirements:
• Current licensure as a LVN/LPN in the state of practice
• Minimum 1 year of experience in a similar sub-acute setting
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
• Basic computer skills and proficiency in electronic medical records systems
