Job Summary

We are seeking a highly skilled and experienced Senior Case Manager to join our team at the Texas Department of Aging & Disability Services. As a Senior Case Manager, you will be responsible for providing advanced case management services to patients and their families, ensuring that they receive the necessary support and resources to achieve their recovery goals.

Key Responsibilities
  • Collect and analyze social assessment information to determine patient needs and develop individualized recovery plans
  • Collaborate with patients, families, and healthcare providers to ensure continuity of care and effective discharge planning
  • Provide education and support to patients and their families on mental health issues, social skills, and community resources
  • Advocate for patient rights and ensure that they receive fair and respectful treatment
  • Participate in quality assurance activities and contribute to the development of policies and procedures to improve patient care
Requirements
  • Master's degree in Social Work or related field
  • Minimum 2 years of experience in case management or related field
  • Knowledge of community resources and case management principles
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
Working Conditions

This position requires the ability to work in a fast-paced environment and to prioritize multiple tasks and responsibilities. The Senior Case Manager will be required to work closely with patients, families, and healthcare providers to ensure effective care and support.
