As a key member of our recovery team, you will be responsible for providing advanced case management services to patients, families, and legally authorized representatives. Your expertise will be instrumental in ensuring the delivery of high-quality care and support to individuals in need.

Key Responsibilities:
  • Collect and analyze social assessment information to inform individualized recovery plans
  • Develop and implement interventions to aid patients in achieving their recovery goals
  • Provide patient advocacy and support to ensure individuals' rights are respected and protected
  • Collaborate with the recovery team to develop and implement discharge plans
  • Assist Case Managers IIs in completion of job functions as needed
Requirements:
  • Knowledge of case management principles, objectives, standards, and methods
  • Ability to communicate effectively both orally and in writing
  • Ability to collect and analyze program data for quality and outcome monitoring
  • Ability to assess patient needs and coordinate patient services
  • Ability to plan, assign, and/or supervise the work of others
