Job Description:

We are seeking a highly skilled and compassionate Case Manager IV to join our team at the Texas Department of Aging & Disability Services. As a key member of our recovery team, you will play a vital role in helping individuals achieve their recovery goals and live fulfilling lives.


Key Responsibilities:
  • Collect and analyze social assessment information to determine individual needs and develop personalized recovery plans.
  • Collaborate with patients, families, and healthcare providers to ensure seamless transitions and continuity of care.
  • Provide education and support to individuals and their families on mental health, social skills, and community resources.
  • Advocate for individual rights and ensure that patients receive fair and respectful treatment.
  • Participate in quality improvement activities and contribute to the development of best practices in case management.

Requirements:
  • Knowledge of case management principles, community resources, and mental health services.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to analyze data and make informed decisions to improve program outcomes.
  • Strong problem-solving and critical thinking skills, with the ability to adapt to changing situations.
  • Ability to work independently and as part of a team, with minimal supervision.
