Advanced Practice Provider Primary Care Specialist

OSF HealthCare is seeking a skilled Advanced Practice Provider to join our Primary Care team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with our physicians and staff to deliver exceptional patient outcomes.
Responsibilities:
Perform patient assessments and develop treatment plans based on current scientific research and evidence-based practices.
Provide education and counseling to patients and their families on health and wellness topics.
Collaborate with our interdisciplinary team to coordinate patient care and ensure seamless transitions between care settings.
Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.

Qualifications:
Master's degree or higher in nursing or physician assistant studies from an accredited program
Current Illinois license as a nurse practitioner or physician assistant
Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
Minimum of two years of experience as an APP in primary care or a related field
Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
Proficiency in electronic medical record (EMR) systems
