Job Title: Nurse Practitioner Virtual Care Hybrid

We are seeking a highly skilled and experienced Nurse Practitioner to join our team in a hybrid virtual care role. As a Nurse Practitioner Virtual Care Hybrid, you will leverage administrative support to deliver patient care services through virtual technology.

Job Summary:

The Nurse Practitioner Virtual Care Hybrid will work in collaboration with a dedicated team of professionals to provide holistic, evidence-based care to patients. You will be responsible for accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.

Key Responsibilities:
  • Provide patient-centered quality and safety by evaluating primary care, acute, chronically ill, and transitional care patients.
  • Provide healthcare education and counseling, and disposition planning for patients ranging in age 18 months and above.
  • Document all patient care within an EHR according to Client policies and procedures.
  • Collaborate with pharmacy and front store colleagues to provide a complete patient experience.
  • Deliver excellent customer service and tailor communication style to effectively influence quality outcomes and patient needs.
  • Resolve conflict using appropriate management techniques and cultivate and maintain positive relationships among practice employees, colleagues, and external partners.
  • Remain accountable to managing business needs, including budget, payroll, inventory, billing insurance, and payment collection.
  • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice.
Requirements:
  • Minimum of two years of medically-relevant experience or equivalent.
  • Effective verbal, written, and electronic communication skills.
  • Outstanding organizational skills and ability to multi-task.
  • Initiative, problem-solving ability, adaptability, and flexibility.
  • Ability to work without direct supervision and practice autonomously.
  • Proficient with information management and technology.
  • Capacity to collaborate with professional colleagues as necessary to provide quality care.
Education:

Must be certified as FNP and PMHNP in at least one of the following states: AK, AZ, CA, HI, IL, MN, MT, NM, OR, UT, WA, MO.

Completion of a Masters Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role required.
