Job Title: Certified Nursing Assistant - In-Home Care Specialist

ConcertoCare is seeking a Certified Nursing Assistant to join our team as an In-Home Care Specialist. As a key member of our care team, you will provide high-quality patient care in the comfort of their own homes.

Job Summary:

We are looking for a compassionate and skilled Certified Nursing Assistant to deliver in-home care to patients and provide virtual clinical care support under the general supervision of a registered nurse or licensed medical provider.

Key Responsibilities:
  • Conduct in-home visits to provide direct patient care, including patient observation, collection of vital signs, and administration of health screenings.
  • Facilitate virtual visits with remote licensed medical providers.
  • Provide virtual support for patients, including pre-visit chart prep, tracking diagnostic and referral results, and managing quality measure orders and results.
  • Coordinate and support patient care by performing outbound calls and tasks as directed by a supervising clinical practitioner.
  • Maintain up-to-date electronic health records for vaccination history, preventative care, and other quality measures.
  • Deliver exceptional customer service to patients, caregivers, and clinical partners.
  • Perform additional clinical procedures, including specimen collection and diagnostic point-of-care testing, as permitted by State scope of practice regulations.
  • Accurate and timely documentation of all patient care support activities in the medical record.
  • Participate in patient engagement and experience initiatives and efforts.
  • Foster relationships with patients, caregivers, and partnering organizations.
Requirements:
  • Current certification as a Certified Nursing Assistant in the state of Rhode Island.
  • At least one year of experience in the last 24 months as a Certified Nursing Assistant or Patient Care Technician in a hospital, home health/hospice, or skilled nursing/long term care facility.
  • Current certification in Basic Life Support (BLS) required.
  • Ability to conduct patient home visits, with prior in-home experience preferred.
  • Average to advanced computer and software skills required.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation.
