Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team at UnitedHealth Group. As a Home Health Care Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Conduct thorough assessments of patients to determine their care needs and develop personalized plans of care.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed.
- Document patient information, including progress notes, medication lists, and other relevant data.
Requirements:
- Current RN licensure in the state of Texas.
- Minimum 1 year of experience in home health care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in electronic medical records and other relevant software systems.
