MIH Community Healthcare Specialist

Job Summary: Mercy Flights Inc is seeking a skilled Community Health Worker to join our team. The successful candidate will work closely with our MIH team, primary care teams, and social services agencies to provide short-term care coordination and connection to resources and support to program clients.
Key Responsibilities:
- Provide quality patient care in accordance with clinical operating guidelines and policies.
- Maintain comprehensive electronic client files, including client notes, release of information, assessments, and other medical documents.
- Complete patient care reports promptly, adhering to Oregon State law and company protocols.
- Attend mandated company training functions and required education for the program and company.
- Build and maintain positive working relationships with clients, Allied health providers, nurse case managers, agency representatives, supervisors, and office staff from diverse cultural and socioeconomic backgrounds.
Qualification Requirements:
- A High School Diploma or equivalent is required.
- Oregon Driver's License.
- CPR certification.
- An associate degree in healthcare or human services is preferred, along with a combination of education and experience.
- Any combination of one year of healthcare/social services experience and/or education is required; two years is preferred.
- Experience in the substance abuse and/or mental health field is preferred.
- Community Health Worker certificate is required to obtain within one year of start date.
